External beam radiation is the original form of radiation used for the treatment of prostate cancer. With external beam radiation, the source of the radiation is outside the body and aimed towards the tumor inside the body. Inevitably some surrounding tissue is damaged as well.

The serious side effects off external beam radiation are considerable, and in my view to be avoided, because the evidence is very strong that irradiating the prostate does not improve the chances of survival. There is little question that external beam radiation reduces the quality of life for those men who have it.

Despite the advent of other radiation therapies such as brachytherapy and proton beam therapy claiming to be less invasive and less damaging, there is no evidence that these later technologies are any better in improving survival rates, cancer recurring rates, urinary problems, bowel problems, impotence and erectile dysfunction.
External Beam Radiation
Three-dimensional conformal radiotherapy (3D-CRT) and Image-guided radiation therapy has essentially replaced conventional external beam radiation therapy (EBRT) in the management of early-stage prostate cancer. 3D CRT is said to reduce the side effects by 30% compared to conventional external beam radiation.

The cancer recurrence rates with external beam radiation at 5 years, approached 35%-40% in most series; The cancer recurrence rates at 10 years was 60%-70%. At 15 years, failure rates continued to increase to70%-80%.

Other side effects of external radiotherapy can be diarrhoea, irritable bladder, pain passing urine and sore skin in the treatment area.

Long term side effects can come on months or years after treatment. In most cases these effects are permanent and can get worse over time. A straining feeling in the back passage, called proctitis is the most common long term side effect affecting up to 60% of patients.

Up to 8 out of every 10 men (80%) will no longer be able to get an erection after external radiotherapy treatment for prostate cancer.

A narrowing of the tube from the bladder to the penis (the urethra) can cause difficulty passing urine in many men who have had external beam radiation.
